A Case of Paralysis of Lower Extremity with Hypokalemia Due to Aldosterone Producting Adrenal Cortical Adenoma

Abstract

|
A 32 year-old-man was admitted because of paralyesis of lower ext5remity and hypertension. Inappropriate kalliuresis in the presence of hypokalemia was noted with increased aldosterone level in serum. Plasma renin activity was not increased
during
provocative test and aldosterone level failed to be suppressed during saline loading test. In postural test, aldosterone level was paradoxically decreased during 4 hours of ambulation.
Computerized tomography of abdomen showed well-defined ovoid mass of low density in the right adrenal gland. Right adrenalectomy was done and patholotgic findings showed golden yellow colord adenoma being composed of a large clear cells with
foamy
cytoplasm. Postoperatively, there were reversal of hypertension and hypokalemia.
We present a case of paralysis of lower extremity with hypokalemia due to aldosterone producing adrenal cortical adenoma with brief review of literature.
